Dear Superintendent,

This form is being submitted to notify you of my child's intent to participate in interscholastic activities as a charter, home school, or private school student. I hereby attest that he/she was a homeschool student for a full academic year prior to participation in the activity, and resides within the attendance zone of the school for which the student will participate. I understand that he/she must meet all eligibility requirements set by the South Carolina High School League and the District, with the exception of the District's school or class attendance requirements, or the class and enrollment requirements of the associations administering the interscholastic activities.
